Requests We Process
- Attorneys and law firmsWith a signed client authorization, subpoena, or court order.
- Insurance and claims administratorsIncluding disability, workers' compensation, and Social Security.
- Physicians and healthcare providersFor continuity of care, referrals, and coordination of treatment.
- Government agencies and courtsWith proper legal documentation.
- Schools and employersOnly with the client's signed authorization specifying what may be shared.
- Clients and their representativesClients, parents or guardians of minors, and personal representatives.

Complete requests are processed first.
- The client's full legal name and date of birth
- A signed, dated HIPAA-compliant authorization naming Edify Counseling Group
- A subpoena or court order, if the request is legal in nature
- The specific records and date range you need
- Photo ID, if you are the client or the client's parent, guardian, or representative
Please Note
Records Policy
- Psychotherapy notes are kept separate from the clinical record and require their own specific authorization.
- We release records as documented. Requests for letters, opinions, forensic evaluations, or court testimony are not records requests and are handled separately.
- Copy fees may apply to third-party requests as permitted by Florida law. You will be told of any fee before records are released.
- Only the minimum necessary information is released, limited to what the authorization allows.
